Procedures for Requesting Disclosure of Personal Data Held by Our Company

  1. Requesting to Disclose your personal data held by our company 
    (1) Based on the Personal Information Protection Act, customers (or their proxies) can request our company to disclose their personal data held by our company and the records on provision of such data to third parties.

    (2) Please be aware of the following when making a request:
    • "2. Request Procedures” outlined below must be followed.
    • We ask for your cooperation to provide the information necessary to identify the personal data and the records on third-party provision subject to disclosure.
       
    (3) If any of the following applies as a result of investigation, we will respond to you that we cannot respond to all or part of your request:
    • When there is a risk of harming the life, body, property, or other rights and interests of the customer or a third party
    • When there is risk of significantly hindering the proper execution of our company's operations
    • When it would result in violation of laws and regulations
    • When the information subject to disclosure does not correspond to “personal data held by our company”.
    • When check and recording on third-party provision of the data subject to disclosure are not obliged by laws and regulations.
       
  2. Request Procedures
    Request method
    • Please call the reception desk indicated below by phone. We will send you the necessary forms such as the Request Form by post. 
    • Please send back the completed Request Form and the necessary documents to the reception desk.

    Request Form
    • Please fill in the “Request for Disclosing Personal Data Held and Records on Third-Party Provision” specified by our company and send it to the reception desk.
      Note: If there are any deficiencies on the Request form, we may not be able to accept your request. Please fill out the form without omission of entry. We may call you during the day to confirm the details of your request.
    Customer identification


     
    • When submitting your request, we will verify your identity with the following documents. Please attach copies of two items from the following:
      • Driver's license, passport, health insurance eligibility certificate (*1), pension book, welfare book, residence card, special permanent resident certificate (Only those that are within the validity period or currently effective are accepted.)
        (*1) The previous types of health insurance cards will be accepted as identification documents until December 1, 2025.
      • Seal registration certificate (registered seal must be affixed on the Request form), copy of resident certificate 
        (Only those issued within the past six months are accepted.)

      Note: 
      1. We cannot accept identification documents that do not have your "name," "address," and "date of birth."
      2. We will protect your privacy adequately in handling your personal identification documents you submit, and dispose of them after a specified period.

      Proxy If the request is made by a proxy, the proxy’s personal identification documents indicated above are also required in addition to those of the customer.
    Check of the proxy authority If the request is made by a proxy, we will verify the proxy authority. Please also submit one of the following documents according to the proxy’s relationship with the customer: 
    <Legal Guardian>   Family register (Koseki Tohon) 
    <Adult Guardian>   Certificate of registered matters for adult guardianship 
    <Voluntary Proxy>  "Power of Attorney" prescribed by our company and the information subject’s seal registration certificate

    Note: 
    1. Only original documents issued within the past six months shall be accepted.
    2. Please affix the customer's registered seal (jitsuin) on the Power of Attorney.
    Disclosure method
    • Our bank will disclose personal data held by our company and its third-party provision records using either of the formats indicated below. When filling out the "Request for Disclosing Personal Data Held and Records on Third-Party Provision," please specify either of the following formats:
      (1) Electronic format: We will store the PDF file including the data in a USB memory device specified by our company and send it to your address via restricted delivery mail. 
      (2) In writing: We will send the Response document to you via restricted delivery mail. 
      Note: "Restricted delivery mail" is a service where the post office delivers the mail by verifying the identity of the receiver with the addressee specified on the mail before handing it.
    Fee
    • The following fee must have been paid when we receive the Request Form.
      Amount
    • 1,650 yen (tax included) for each copy of the response document issued.
    • If you choose the electronic format, an additional 1,100 yen (tax included) is required.
    Payment method  
    • Registered mail for cash
  3. Response document
    We will send you the response document in the format you have specified within 30 days after receiving the Request Form, etc. in principle.
     
  4. Inquiries
    (1) If you have any inquiries regarding this procedure, please call the reception desk mentioned in the above chart by phone.
    (2) If you are a customer of a group company of the Mizuho Financial Group, both inquiries and the disclosure process shall be handled by the company with which you have transactions.

 

 

Procedures for Requesting Notification on the Purpose of Use of Personal Data

    (1) In accordance with the Personal Information Protection Act, customers (or their representatives) may request our company to notify them of the purpose of use of their personal data we hold.
    (2) When making such a request, we ask for your cooperation in providing the necessary information to identify the personal data.

Procedures for Requesting Correction, etc. of Your Personal Data Held by Our Company

  1. Requesting for correction, etc. of your personal data held by our company
    (1) In accordance with the Personal Information Protection Act, customers (or their proxies) may request to correct their personal data held by our company.
    (2) Correction, etc. (correction, addition, or deletion) can be requested if "the personal data held by our company” is not factual." However, please understand that we may not be able to comply with your request in the following cases:
    • When special procedures for correction, etc. are prescribed by law
    • When correction, etc. of your personal data is not necessary to achieve the purpose of use of the relevant personal data

    Cases when we decline the request for termination. of use, etc. 
    • In the following cases, we will decline requests on termination of use, deletion, or third party provision of your personal data. When we decide to decline the request to terminate the use, deletion, or third party provision, we will inform you of the fact and explain the reason.
      • When the content of the request for termination of use, etc., is not factual, or when the authenticity of the request cannot be confirmed.
